FIRST TIME VISIT

Our goal at Spring Mountain Dental is to provide you the highest quality dental care possible at an affordable price. Our reputation for honesty and integrity has become a hallmark in Las Vegas. We understand visiting the dentist can be a daunting experience for some, but we pride ourselves on putting our patients at ease and making them comfortable with our procedures. To that end, we will fully discuss necessary dental procedures and those which are not. Available options will be discussed to assist you in making the best informed decision.

There are a few things that you should keep in mind when visiting Spring Mountain Dental for the first time.

FIRST:

Please check to see that we are participating in your insurance program. You can view the insurance programs that we are currently accepting here. If you are unsure if we are participating in your insurance program, you can verify by calling us at (702) 368-3854. If you do not have insurance, we do offer affordable dental care with a FREE consultation to discuss plans we have available to assist you in receiving affordable dental care. SECOND:

If you wish to speed up the process of your paperwork, we recommend you visit our Patient Forms pageand download the basic form. Once you have downloaded the form, print them, fill them out, and bring them with you to your first appointment. If you wish to fill the forms out at or Las Vegas Dental Practice, please arrive a few minutes early for your appointment providing the time necessary to complete them.

THIRD:

Make sure you have all relevant information, such as your dental insurance information, that you will need to complete the necessary forms. You will also have to impart your medical history as well. FOURTH:

Please inform the dental professional assisting you if you:
• Been diagnosed with any diseases or conditions.
• Taking any medication. This includes both prescription and over-the-counter medication.
• Have any allergies.
• Believe you have a new cavity.
• Feel lumps inside your mouth.
• Find that your teeth have become sensitive.

FIFTH:

If you have any fears of undergoing dental procedures, please tell us. Many misconceptions have arisen over visits to the dentist and a frank discussion about your fears will most likely alleviate many of those feelings.
